Supply Chain Evaluation Tool

This AHRMM tool has two practical supply chain evaluation templates featured in the Customer Service in Health Care Supply Chain: Certificate Course, released in January 2019.

About the Tool:

The tool has two sample evaluation templates to gather information on the performance of the supply chain department in a hospital or health system. The first evaluation template, Supply Chain Service Survey, should be completed by departments working with the supply chain in a hospital or health system. 

The second evaluation template, Supply Chain Rounds Report, should be completed each time a visual round is finished in the supply chain department. This report can be left with the supply coordinator or department manager. The report informs key stakeholders of any timely information or issues found within the stockroom, such as stock outs, bin or label issues, special orders or any other potential areas of concern for the supply chain.

These evaluation templates are not intended to be all-inclusive. It's recommended providers use these templates to develop their own surveys, suited for their individual organizations.
